1. Arrange for someone(s) to supervise the survey - concerned citizens.  They should arrange for dispersing, collecting and tabulating results before Town Meeting.  
2. Talk to Superintendent about the survey so they are aware of it. 
3. Prepare bags to collect the surveys.  Put a small weighted item in the bottom.  
4. Print copies of the survey and cut them apart.  Attach a string to the bag handle and a paper clip to the other end to hold about 25 surveys on the table next to the bag.
5. Arrange to put bags to collect surveys in at public locations - Town Hall, Library, School...
6. Collect surveys with time to tally and report results at Town Meeting, School Board Meeting, and newspaper/ Front Porch Forum
